Registration Request Form for ITRE Training

• When you have completed the form, use "submit" button only once.
  Do not use the Enter/Return key.
• After you "submit", print a copy of your Web confirmation page to MAIL with your payment. An e-mail will be sent with the same confirmation information for your records.
• Mail your payment to:

• Make check payable to: North Carolina State University.
• We accept cash (do not mail), purchase orders, money orders, and checks for payment.

• To register another participant, use the browser's back button and form information will be retained

This is a registration request for the training courses listed below. Attendance is limited. Confirmation of your registration is dependant upon seating availability based on a first-come, first-serve basis. If class is full or cancelled due to low enrollment, you will be notified immediately and asked to reschedule.

A confirmation letter, map and list of accommodations will be mailed to you two weeks before the workshop. You will also get an immediate confirmation on your screen once you are registered in addition to a confirmation email. If you don't get these immediate Web and email confirmations, your registration has not gone through and you will need to register again.

The cost of registration covers materials provided as well as break refreshments. Unless otherwise noted, lunch is not provided.

Information on lodging and area restaurants will be mailed to you along with your confirmation letter within a week after receipt of registration. Once your request has been processed, telephone verification is not necessary unless you have not received registration materials one week prior to the course. Parking passes may be obtained at the Centennial Campus Visitor’s Information Center. As of October 2005, the university charges $2 daily parking, confirm with your course registrar how to obtain a parking pass, BEFORE going to the parking booth and paying the fee out of your own pocket. Pre-purchased parking passes, if any, are arranged by individual course registrar for each course.

Persons with disabilities who require special accommodations or auxiliary aids should contact ITRE ten (10) days prior to the workshop date so that necessary and reasonable accommodations can be made.

Cancellation

If you pre-register but cannot attend, please provide notice of cancellation at least five (5) days prior to the workshop in order to receive a full refund. If you cancel with less than five (5) days’ notice, you will be charged one-half the original class fee. A substitute may take your place. Classes are subject to cancellation due to low enrollment; you will be notified.
